Intro: Steroid`s Problem

Androgens are a class of hormones that serve as ligands (a molecule linked to another molecule) that bind to cellular androgen receptors (AR). The androgen receptor is involved in a complex signal transduction pathway that ultimately results in greater expression of specific genes. All anabolic steroids offer their muscle building properties through this binding to the androgen receptor.

The androgen problems with anabolic steroids or syntetic derivates of testosterone arise from their many undesirable harmful side effects or pharmacokinetic properties. Unfortunately, steroids effects on all androgen receptors in the body, therefore, where it is the opposite undesirable, and just from this spring large part of their adverse effects.

Hepato-toxicity: The next great problem of oral / tablets steroids is their very high liver toxicity: Unfortunately, clear steroids - syntetic derivates of testosterone has very poor activity and effects when taken orally. The first synthetic alteration that scientists utilized is known as 17-alpha alkylation. 17-a alkylation involves the addition of an alkyl group (methyl or ethyl) to the alpha position of the 17 carbon of the steroid backbone. The alkylation at this position prevents the major route of androgen deactivaton – oxidation to a 17-keto steroid – from taking place. This allows a large part of the steroid to avoid liver first pass metabolic degradation. Examples of 17-alpha alkylated steroids are Methyltestosterone and Norethandrolone:

While 17-alpha alkylation is a very effective means of rendering steroids orally active, it suffers from a serious drawback: These steroids are all to some extent toxic to the liver. Some are more toxic than others, but they all have been associated with this problem. Several cases of liver cancer have supposedly been linked to 17-a alkylated steroids.
